Our Responsibilities
Root Integrative Medicine is required by law to maintain the privacy and security of protected health information, provide this notice of our legal duties and privacy practices, notify affected individuals following a breach of unsecured protected health information when required, and follow the notice currently in effect.
How We May Use and Share Information
We may use or disclose protected health information for treatment, payment, and healthcare operations. Examples include coordinating care with another provider, billing for services, maintaining records, quality review, licensing and compliance activities, and operating the practice. We may also use or disclose information when permitted or required by law, including for public health, health oversight, legal proceedings, law enforcement, workers’ compensation, or to prevent a serious threat to health or safety.
Your Written Authorization
Uses or disclosures not otherwise described in this notice generally require your written authorization. You may revoke an authorization in writing, except to the extent that action has already been taken in reliance on it.
Your Rights
- Request access to or a copy of your health information.
- Ask us to correct health information you believe is inaccurate or incomplete.
- Request confidential communications by a particular method or at a particular location.
- Ask us to limit certain uses or disclosures, although we may not be required to agree.

- Obtain a paper copy of this notice.
- Choose someone with legal authority to act for you.
Complaints
You may contact Root Integrative Medicine if you believe your privacy rights have been violated. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
