Medical Information
Do not submit symptoms, diagnoses, medical history, medications, insurance information, or other private medical information through the general website inquiry form or ordinary email. Current and prospective patients should use the secure OptiMantra patient portal when asked to provide health information.
Service Providers and Links
We use service providers, including Netlify for website hosting and OptiMantra for secure patient requests, scheduling, forms, and patient communications. Those providers process information under their own terms and privacy practices. The website may also link to third-party sites that Root Integrative Medicine does not control.
Data Security and Retention
We use reasonable administrative and technical safeguards appropriate to the information collected. No internet transmission or storage system can be guaranteed to be completely secure. Information is retained only as reasonably necessary for the purposes described above and applicable legal obligations.
Your Choices
You may request that we update or delete information submitted through the general website form, subject to legal and operational requirements. You may also opt out of nonessential marketing communications using the instructions in those messages.
